3. Name and list of machinery and equipment with the installed capacity.

4. List of the category of food to be manufactured

5. Authority letter for manufacturer giving a responsible person name or address

6. Source of raw material

7. PA/NOC document issued by FSSAI

8. Form IX

9. Certificate from the ministry of tourism

10. Possession of premises proof

11. Food safety management system certificate

12. Document for proof of turnover

13. NOC from the local body or municipality.

Documents for obtaining State License

1. Form B completed and signed

2. List of partners/ Directors with address, contact details, and photo id proof

7. Copy of License and NOC from the Manufacturer
